While patient care is our first priority, there are many federal and insurance guidelines we must follow. We apologize in advance for any inconvenience to our patients.
Please bring with you to your appointment the following:
Photo ID with current address. This is federally mandated by the “red Flag” law which helps protects you from identity theft.
- A copy of your insurance card: All insurance companies require that we keep a copy of your insurance card on file.
- Payment for copays/deductibles. You will be required to pay your co-pay or deductible at the time of service. If you do not have your payment, your visit will be rescheduled.
- Please bring to your visit a copy of your medications with the doses and prescriber.
- New patients please arrive 10 minutes before your appointment time. Patients arriving more than 10 minutes after their appointment time will be rescheduled
